TRT: What to Expect in Your First 90 Days
Labs, dosing, and honest timelines — what a well-run testosterone replacement program actually looks like.
Start with labs
A responsible TRT program begins with a full panel — total and free testosterone, estradiol, SHBG, CBC, CMP, lipids, and PSA where appropriate. Symptoms alone are not enough.
Week 1–4
Once your physician confirms clinical hypogonadism and rules out reversible causes, therapy typically starts with a conservative dose. Most patients notice modest energy and mood improvements in the first month.
Week 4–8
Follow-up labs guide titration. Sleep, training, and libido often improve here. Side-effect management — estradiol control, hematocrit monitoring — is part of good care.
Beyond 90 days
Body composition changes emerge gradually. Ongoing labs every 6–12 months keep therapy safe and effective. TRT is a commitment — a good clinic makes it a simple one.
